Iran defender Ezatollah Pourghaz was forced to play in goal for the last twenty minutes after the visitors had made all their substitutions.
Team Iran missed four players and two staff because of visa issues in the match against Guam.
The win lifted unbeaten Iran to 14 points, three clear at the top of the Group D standings.
“We did well, so I'm pleased to have won three points against Guam. We still are at top of the group and it can help us in two remaining matches against India and Oman. I am satisfied with my players’ performance and congratulate them,” Carlos Queiroz said in the post match news conference.
“I respect Guam players since they fought the 90 minutes. I wish them very best for the future,” the former Real Madrid coach added.
Iran will host India and Oman in March.
Only the eight group winners and four best runners-up will progress to the third round of Asian World Cup qualifying and also book a spot at the 2019 Asian Cup.
